Service Forms

By means of the following forms, you may request the Superintendence of Casinos  for orientation and response to inquiries, reports or claims that may arise due to the operation and /or exploitation of casinos, as well as file complaints, suggestions and compliments regarding the services rendered by the SCJ. 

In your application form refers to a claim, we inform you that said form, must be filed in first instance before the casino in which the matter of the claim was originated, within a maximum period of 15 calendar days from the date of said issue. 

If you have already filed your complaint and are not satisfied with the Casino´s response you may file a revision request before the Superintendence. Said appeal must be filed within a 20 business day period, from the date of the casino´s response, attaching a copy of the response issued by the casino.

IMPORTANT
Please remember that according to the Casino Act, the Superintendence of Casinos shall not regulate concessions of Casinos in Arica, Iquique, Coquimbo, Viña del Mar, Pucón, Puerto Varas and Puerto Natales, until December 31st, 2015. Until this date, the municipalities of the cities of the concession will be in charge of supervision and control of the respective Casinos and their operation.
